CSPS Reproduction Request Form
If you wish to reproduce any item or any portion of any item (image, document, article, data, etc.) from the website of the Council on Surgical and Perioperative Safety, with publication (including e-books, magazines, journals, etc.), broadcasting, exhibition/presentations or posting on another website and any other medium you should take the following procedure.
- Fill out the below form online.
- Submission request will be confirmed by e-mail.
- Please note that some requests may be refused or may not be accepted.
- Permission or denial will be received by submitter within 30 days.
